We participate with most local and many national insurance plans. However it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive. We are not currently contracted with Multiplan and PHCS. Although we are not providers, we will see you using your out-of-network benefits if you have them, or even if you don’t have benefits we will see you at a discounted rate and still bill the insurance company on your behalf.

You must provide complete and accurate information about your insurance, as a courtesty to you, we will submit claims to your insurance carrier for payment. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. It it imperative that you inform us immediately about any insurance changes. You must bring and updated insurance card to your appointment otherwise you will be considered a cash patient and be responsible for payment at the time of service,
