Billing Information
We participate with most local and many national insurance plans. However, it is your responsibility to understand whether your insurance has limits on the doctors you can see, or the services you can receive.
You must provide complete and accurate information about your insurance, as a courtesty to you, we will submit claims to your insurance carrier for payment. Depending on your insurance coverage, you may be responsible for co-payments, co-insurance, or other deductible amounts. It it imperative that you inform us immediately about any insurance changes. You must bring and updated insurance card to your appointment otherwise you will be considered a cash patient and be responsible for payment at the time of service.
We collect at the time of the visit based on a complete and thorough verification process. We take all payment methods except American Express, and we do not accept cash over $20.00 and the change must be exact.
