Office Policy

Appointments

Appointments can be made by calling our office during normal business hours.

Our phone hours are 8:00 to 5:00 Monday – Friday.

New patients must bring their insurance card, co-payment (if any) and present a driver’s license.

Anyone under 18 years of age must be accompanied by a parent that can sign financial forms.

We will not see anyone under 18 years of age without a parent present.

A no-show fee will be charged from 35-50 dollars, depending on the type of appointment.

We do charge for telephone appointments with the doctor.

After Hours & Emergency Care

If you are experiencing a life-threatening emergency dial 911 or head to the nearest emergency room. For all other non-life threatening emergent calls, an on-call physician is available after hours a day, 7-days a week by calling our physicians exchange at 714-447-5039. If you have a general question that is not an emergency, we ask that you wait until regular office hours to call and speak with someone which are Monday – Friday from 8:00 – 5:00.

Emergency

Prescription Renewals

Prescriptions are renewed during normal office hours. Have your pharmacy electronically request a refill or new prescription. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Disability, FMLA and other forms

Please talk to your medical assistant in regard to your forms request. We make every effort to prepare your forms within 1 week but it may take longer. There is a charge of $20.00 per form, and $10.00 for extensions.

Financial

All office visit co-payments and co insurance are payable at the time of service. We accept cash, checks, Visa, MasterCard, Discover and debit cards. Please contact our billing office should you have any financial questions or concerns.

Medical Records Release

There is a charge of $15.00 for medical records requests. If you wish for billing documents as well, it will be an additional $15.00 for a total of $30.00. We do require a 10-business day notice.
